New Account Application

Establish an Account in the BlackRock CollegeAdvantage 529 Savings Plan.

Incoming Rollover Form

Complete this form if you are rolling over an account with assets from another qualified 529 Savings Plan, an UTMA/UGMA, or qualified U.S. savings bonds along with your New Account Application. This form may also be used in the future to rollover other assets into your existing BlackRock CollegeAdvantage 529 Account.

Payroll Deduction
Authorization Form

Digital Form: Fund your account by having your employer deduct from your payroll and send the contribution directly to your BlackRock CollegeAdvantage 529 account.

To begin a payroll deduction plan, check with your company to make sure it offers this service. If your company has not yet established a corporate payroll deduction plan, they will need to complete the Employer payroll deduction form.

Establish a Company
Sponsored 529 Plan

Digital Form: Employers looking to establish a company-sponsored 529 plan to help your employees to invest in the BlackRock CollegeAdvantage 529 Savings Plan can complete this form. It will explain the different ways your employees can contribute and allows us to establish your plan. Talk with your Financial Professional about the benefits of setting up a company-sponsored plan.

529 Distribution Request for Direct Rollover to a Roth IRA

Digital Form: Distribute from your 529 plan for a direct rollover contribution to a Roth IRA owned by the 529 account beneficiary.

Account Maintenance Form

Digital Form: Change your address, add or change bank instructions or update your automatic investment plan instructions.

Change of Name Form

Digital Form: Update your name due to marriage, divorce, or other reasons.

Change of Registration Form

Update the Account Owner, Successor Owner, Beneficiary, or transfer the account into the name of the Beneficiary with. This form is used to make ownership changes to your account(s).

Contribution Slip

Digital Form: Contribute to any existing BlackRock 529 account.

Reallocation Request Form

Digital Form: Reallocate your Investment Options, or to establish a Systematic Reallocation. You may only change your Investment elections once per calendar year. If you plan on setting up a Systematic Reallocation when you open your Account, you must submit this form along with your New Account Application to avoid using up your annual change.

Withdrawal Request Form

Digital Form: Request a redemption from your BlackRock CollegeAdvantage 529 Account to pay for qualified higher education expenses.
